Support for Mental Health and Developmental Disabilities

 

 

FAMILY SUPPORT (Any age; individuals must be living with a family member (parent, grandparent, sibling)

  • Coordinated and caring support to help navigate information, services, and financial challenges.

IN HOME SUPPORTS (Ages 3-22)

  • Personal care, basic living skills (eating, drinking, toileting, personal hygiene, dressing).
  • Mobility and physical functioning.
  • Communicating (including with the use of technology); learning to make choices/show preferences, and have opportunities to satisfy those interests.

PARTICIPANT DIRECTED AND MANAGED SERVICES (PDMS)

  • Provides individuals and/or their families the option to design, manage and direct their support services using pre-authorized Medicaid funds.
  • Participants choose their own staff, decide how much to pay them, and determine their schedule.
  • Service coordination; family support; personal care/residential; supported employment; independent living; respite; specialty services; assistive technology; home and vehicle modifications; and crisis response.

HOME CARE / RESIDENTIAL SERVICES

  • Home care connects individuals with a qualified family who is willing to share their home.
  • Providers share their homes, lives and families, giving care in a supportive, home environment.
  • Provides your family member with the opportunity to learn life skills, develop greater self-esteem, and live a more interdependent life within his or her community.

VOCATIONAL SERVICES / DAY SERVICES / COMMUNITY SUPPORT SERVICES (Ages 18 and above)

  • Assists adult individuals to become vital members of their communities.
  • Vocational coordinators and direct support professionals, work with employers in the area to cultivate work environments accepting of diversity.
  • Provides your family member with the opportunity to learn life skills, develop greater self-esteem, and live a more interdependent life within their community.

SCHOOL TO COMMUNITY-BASED TRANSITION SERVICES – AGES 14 TO 22

  • Support and guidance with the planning process regarding transition from school programs to adult supports including identifying individual needs.
  • Assistance with exploring community options and accessing adult benefits, services and other community resources.
